Position Overview
Ultimate Staffing Services is seeking a dedicated and compassionate Outreach Specialist. The Outreach Specialist will play a critical role in enhancing care management services, supporting high-risk individuals by coordinating comprehensive, person-centered care across various systems. This position focuses on improving patient health outcomes by addressing both clinical care and the social determinants of health.
Responsibilities

• Conduct outreach and engagement with Enhanced Care Management (ECM) eligible members using trauma-informed and culturally responsive approaches.
• Complete comprehensive assessments addressing medical, behavioral health, and social determinants of health.
• Assess social needs and barriers to care such as housing instability, food insecurity, and transportation challenges.
• Develop, implement, and update individualized care plans in collaboration with members and interdisciplinary teams.
• Coordinate services across primary care, specialty care, behavioral health, social services, and community-based organizations.
• Facilitate referrals and follow-up for supportive services such as housing support and food assistance.
• Monitor member progress toward care plan goals and adjust interventions as needed.
• Serve as a liaison between members, providers, managed care plans, and community partners.
• Maintain timely and accurate documentation in electronic health records and care management systems.
• Ensure services are delivered in compliance with Medi-Cal ECM requirements and organizational policies.
• Conduct outreach via phone, in-person, or electronic communication to engage referred patients.
• Educate patients and families about ECM to promote understanding, enrollment, and engagement.
• Support ECM data collection and reporting requirements, including tracking outreach, enrollment, and resource connection outcomes.
• Track and report required grant metrics, outcomes, and performance measures.
• Assist and support internal and external audits, quality improvement initiatives, and program evaluations required by the grant.
• Demonstrate competence with the mission, vision, and values of the organization in providing quality services to the community.
• Other duties as assigned, maintaining confidentiality and professionalism at all times.

Qualifications

• High School Diploma or equivalent required; Associate degree or Medical Assistant Certificate preferred.
• A minimum of 3 years of relevant experience focusing on customer service and satisfaction.
• Valid driver's license.
• Multi-lingual preferred (English with Spanish, Punjabi, or Portuguese).
• Strong communication, organization, and documentation skills.
• Knowledge of social determinants of health and community resources.
• Ability to work with high-needs and vulnerable populations.
• Proficiency with electronic health records and data tracking systems.
• Proficient with Microsoft products and other IT requirements.

Benefits
Competitive hourly pay ranging from $21 to $22 per hour.
