Position Summary

The Licensed Practical Nurse (LPN) performs nursing functions within the household/community by administering medications, monitoring residents' conditions, and communicating changes to the Registered Nurse (RN) and physicians as needed. The LPN works collaboratively with the interdisciplinary team to ensure regulatory compliance, promote person-centered and resident-directed care, and support a safe, compassionate, and home-like environment for residents.

The LPN serves as a teacher, mentor, and role model for professional nursing practices while advocating for quality clinical care and resident well-being. This position provides routine nursing care and supportive services in accordance with resident assessments, care plans, and individual preferences.

Essential Job Responsibilities

• Administer medications and treatments accurately while monitoring residents for effectiveness and adverse reactions.
• Observe, assess, and report changes in resident conditions to the RN and physician as appropriate.
• Provide direct nursing care and support individualized care plans based on resident needs and preferences.
• Promote Person-Centered and Resident-Directed Care in all aspects of service delivery.
• Ensure compliance with federal, state, and local regulations, including the Nurse Practice Act and professional nursing standards.
• Maintain accurate documentation including medication administration records, resident charting, and RAI documentation.
• Participate in orientation, training, competency evaluations, coaching, and staff education activities.
• Support infection control, resident safety, sanitation, and quality assurance initiatives.
• Foster collaboration, communication, and teamwork within the household/community.
• Build positive relationships with residents, families, physicians, and interdisciplinary team members.
• Assist in problem-solving, conflict resolution, and household operations to promote continuity of care.
• Maintain a welcoming, home-like environment that promotes emotional, psychological, and spiritual well-being.
• Participate in interdisciplinary care planning and implementation of resident care goals.
• Support organizational efficiency through appropriate use of supplies, resources, and staffing.
• Advocate for resident rights, dignity, independence, and quality of life.

Requirements

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in applicable state.
• Current CPR/BLS certification preferred or required per facility policy.
• Previous long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong communication, leadership, and organizational skills.
• Ability to work effectively in a team-oriented environment.
